$1000 Hiring Bonus available JOB SUMMARY The Facilitator
coordinates the development of Child and Family Teams (CFTs),
maximizing the involvement of natural team members and system
partners. The facilitator guides the CFT through the plan
development process using accepted High-Fidelity Wraparound
technologies and techniques. The Facilitator prepares
individualized family plans and coordinates each plan's
implementation, encouraging the maximum use of natural resources
and community-based services; supplementing these with flexible
Wraparound staffing and ethical use of available funding. The
Facilitator functions as the case manager, representing the CFT's
progress through the Wraparound process in written documentation
and verbal presentations. Supports the Mission, Vision, and Values
of the agency. QUALIFICATIONS Education & Experience Bachelor's
degree in a related field At least 4 years approved experience*
*approved experience is experience that qualifies the applicant for
Mental Health Rehabilitation Specialist (MHRS) status in the El
Dorado County Children's Mental Health System of Care Skills &
Requirements Prepared to work independently with flexible hours
that may include evenings weekends and holidays Proven
documentation skills and intermediate level computer skills
including all MS Office programs Adhere to all confidentiality
requirements Bilingual skills are a plus Verbal and written ability
to communicate effectively with colleagues, community partners,
consumers and all levels of management Ability to multi-task and
prioritize projects and assignments effectively Ability to work
cooperatively in a team environment within program/department and
agency wide Ability to work in a mobile, fast paced environment
Ability to use a laptop computer Fulfillment of TB test, background
check clearance, and any other mandatory State/Federal requirements
Valid California driver's license, proof of automobile insurance,
can maintain a driving record acceptable to the agency's insurance
carrier, if applicable to position Must provide and operate a
registered, properly insured automobile, if applicable to position
ESSENTIAL JOB FUNCTIONS Position Specific Writes clear, accurate
and timely progress notes consistent with each contact/visit Works
collaboratively with all programs and staff to provide support as
needed Attends weekly staff meetings, supervision and case
management staff meetings as scheduled Accountable for independent
time management Develops and maintains positive relationships with
agency and community programs Utilizes resources to utilize
behavior interventions and strategies Forms positive, constructive
relationships with children and their families Communicates
effectively with all members of the Child and Family Team Maintains
positive relationships with consumers and customers Identifies
strengths, skills and positive qualities in children and families
Identifies temporary and permanent sustainable resources and
services within family and community environments Assists families
with transitions from resources provided or funded by Wraparound
program to resources and services provided or funded by the family
and community Serves as role model for the wraparound process,
demonstrating particular commitment to family safety, unconditional
care, family voice and choice, team-driven decision-making,
strength-based planning and interventions, flexible services and
supports and community partnership and collaboration Provides
leadership in the overall assessment, development, implementation
and evaluation of individualized wraparound plans Coordinates the
development of Child and Family Teams (CFTs), maximizing the
involvement of natural team members and system partners Attends and
guides the weekly CFT meetings through the plan development process
using accepted wraparound technologies and techniques Documents the
plan development process by recording and distributing minutes of
CFT meetings, writing progress notes and updating plans Prepares
Individualized Family Plans and Safety Plans and coordinates each
plan's implementation, encouraging the maximum use of natural
resources and community based services, and supplementing these
with flexible wraparound staffing and funding Provides other case
management functions as necessary, securing formal resources and
representing the CFTs progress through the wraparound process in
written documentation and verbal presentations to both internal and
external partners Maintains accurate and timely records Seeks out
clinical information and advice as needed, accepts constructive
criticism and reasonable recommendations from supervisors Mentors
new Specialists Becomes certified as a High Fidelity Wraparound
Facilitator within 1 year of date of hire Ability to work flexible
hours to meet the needs of the families Participates in the agency
24 hour on-call programs Agency Specific Performs all duties in a
manner consistent with the principles and values of agency, while
adhering to applicable professional codes of ethics, the agency's
policies and procedures, contractor requirements, and regulatory
requirements. Model and communicate appropriate positive attitudes
toward the agency's Mission, Vision, and Values Work
collaboratively with all agency programs and staff to provide
support as needed Utilize and maintain calendar with all
work-related details in order to manage time effectively and share
calendar information with coworkers Participate in on-going
training to expand and develop professional skills Perform other
duties as necessary for the agency, as assigned Employment Type:
Full Time Salary: $49,920 - $65,880 Annual Bonus/Commission: No
