Preferred education emphasis in human services, counseling, guidance, social work, etc., ORAA degree AND 2 years of experience working with children or families with complex needs. Preferred education emphasis in human services, counseling, guidance, social work, etc.ESSENTIAL JOB FUNCTIONSPosition SpecificParticipates in the development of individualized family plans with members of the Child and Family TeamWrites clear, accurate and timely progress notes consistent with each contact/visitAttends weekly staff meetings, supervision, and case management staff meetings as scheduledAccountable for independent time managementDevelops and maintains positive relationships with agency and community partnersUtilizes resources to implement behavior interventions and strategiesForms positive, constructive relationships with children and their familiesCommunicates effectively with all members of the Child and Family TeamMaintains positive relationships with consumers and customersIdentifies strengths, skills, and positive qualities in children and familiesIdentifies temporary and sustainable resources and services within family and community environmentsAssists families with transitions from resources provided or funded by Wraparound program to resources and services provided or funded by the family and communityDocuments the plan development process by recording and distributing minutes of CFT meetings and writing progress notesPerforms tasks and services as assigned in individualized plans. Tasks may include but are not limited to:Encouraging and engaging in tasks or activities that support, or " Spark," identified strengths, skills, and positive qualities in children and familiesSupervising individuals and groups of childrenProblem-solving with children and families, including outlining alternatives and predicting consequences while supporting children and families to make good choices for themselves. Teaching specified skills to children and familiesPerforming day-to-day tasks that support family functioning (including transportation, meal preparation, short-term supervision, reminder calls, and other tasks)Modeling appropriate behavior to direct families toward identified goalsParticipating in healthy group dynamics within various settings including family homes, schools, parks and recreation centers, and treatment facilitiesPerforming other tasks identified in individualized plansMaintains accurate and timely recordsFollows all agency policies and proceduresFollows all agency, state, and federal health and safety rules and regulationsAdheres to all confidentiality requirementsSeeks out clinical information and advice as needed, accepts constructive criticism and reasonable recommendations from supervisorsMentors new Family SpecialistsAbility to work flexible hours to meet the needs of the familiesParticipates in the agency 24 hour on-call programsAgency SpecificPerforms all duties in a manner consistent with the principles and values of the agency, while adhering to applicable professional codes of ethics, agency policies and procedures and County Mental Health Plan requirementsModel and communicate appropriate positive attitudes toward the agency Mission, Vision, and ValuesWork collaboratively with all programs and staff to provide support as neededUtilize and maintain Microsoft Outlook calendar with all work-related details in order to manage time effectively and share calendar information with coworkersParticipate in on-going training to expand and develop professional skillsPerform other duties as necessary for the agency, as assignedHiring Range: $19.00 $21.70 DOEEducationPreferredAssociates or better in Human Services or related fieldSkillsPreferredCulturally competentWork IndependentlyAdhere to all confidentiality requirementsComputer skillsAbility to use laptop computerDocumentationFlexibility in scheduleMulti-taskerTime managementCollaborativeWritten and verbal communicationWork mobile, fast paced environmentProblem-solving
