Job Description:
Description Western Sierra Medical Clinic is an engaging,
enthusiastic, mission driven organization dedicated to providing
high-quality primary care to all members of our community. This
includes the delivery of evidence-based preventative care, chronic
disease management, and addressing urgent care needs as indicated
for our pediatric and adult patient population. GENERAL STATEMENT
OF DUTIES Under the general supervision of the Population Health
Manager, the Community Health Worker provides and coordinates
patient linkage with resources and care coordination. Western
Sierra Medical Clinic’s Population Health Outreach is an innovative
program to support and empower identified high risk-high needs
patients to improve their health outcomes. The multidisciplinary
team (RN, MD, MSWs, LCSWs, and Community Health Workers (CHWs))
works together to support patients and address the social
determinants that impact health. These include food, housing,
transport, and financial resources. The WSMC CHW will be a trusted
member of the community in which they will work. This relationship
will enable the CHW to serve as a liaison between health and social
services and the clinic. Community Health Worker Certification will
be offered by WSMC if candidate is not certified. GENERAL DUTIES

other duties as assigned. Advocate for patients and help them
achieve their best health. Be a trusted member of the community. Be
dependable and independent – much of the work will take place in
various settings. Communicate well with patients, document clearly
in the medical record and interface with medical providers to
advocate for the patient. Conduct outreach and enrollment
assessments. Create a care plan that reflects patients’ needs.
Connect patients to resources to meet these needs. Serve as a
liaison between the community and health and social services.
Interface well with clinic staff, community and Care Neighborhood
team. Meet departmental review and documentation standards for work
assignment. Adhere to the policy and procedures of assigned
clinic(s). Build and maintain appropriate relationships on behalf
of WSMC. Support a positive work environment and foster teamwork.
The wage range for this position is $23.00-26.00 per hour.
Qualifications MINIMUM QUALIFICATIONS High school diploma or GED;
at least 1 year of experience related to the duties and
responsibilities specified. Completed degree(s) from an accredited
institution that are above the minimum education requirement may be
substituted for experience on a year for year basis. Community
Health Worker Certification, or willingness to obtain within first
six months of employment DESIRED EXPERIENCE Competent
administrative skills. Flexibility and creativity – new and
evolving program Good communication and consumer relations skills;
ability to work well with a team and independently. Proficiency in
computer operations, navigation skills and software applications.
Sound decision-making skills Professional demeanor Understanding of
managed care services Ability to work collaboratively within a
multidisciplinary team Familiarity with an electronic practice
management system. Computer literate (i.e. logging on to computer,
usage of keyboard and mouse, familiarity with Windows, Microsoft
Word and Microsoft Excel). Ability to type 40 wpm
