About the Role: We’re hiring a full-time Geriatric Physical Therapist to join our interdisciplinary care team in Livermore, CA . You’ll play a vital role in our PACE (Program of All-Inclusive Care for the Elderly) — where your clinical expertise will directly improve mobility, safety, and independence for frail older adults. What You’ll Do: Evaluate the functional ability, mobility, and rehab needs of older adults Develop and implement individualized, evidence-based care plans Collaborate with physicians, nurses, social workers, and caregivers Educate patients, caregivers, and staff on safe mobility and equipment use Supervise PTAs and therapy aides within the care team Evaluates participants’ home environments and makes appropriate recommendations as needed.
