In this role, you will deliver daily patient care in a home
environment. Your responsibilities will encompass a variety of
tasks, including but not limited to: Managing cases and providing
comprehensive care for multiple patients with varying levels of
acuity. Facilitating admissions to home care services. Conducting
post-hospitalization evaluations and recertifications. Overseeing
discharges and providing patient education. Performing dressing
changes and administering IV therapy. Caring for access sites and
delivering general skilled nursing in accordance with established
policies and procedures. Addressing additional needs that may arise
in the home to ensure safety and appropriate care, such as
connecting patients with lifeline services, meals on wheels, and
guidance on public program availability. You will also be
responsible for maintaining accurate and timely documentation using
the Epic computer system. This is a Per Diem position, which means
you will provide visits as needed while adhering to all
departmental policies. Your work will reflect our mission, values,
and service standards. Position Overview: Education: Graduation
from an accredited nursing program is required. Licensure: Must
hold a current state nursing license and possess a valid driver's
license. Experience: Relevant experience is preferred.
Certifications: CPR certification is mandatory. Preferred
Qualifications: While not required, one year of experience in a
