award-winning hospital is a 221-bed acute care facility that
includes an expanded Level III emergency department, a Level III
trauma center and STEMI-receiving center and provides the following
care services: cancer care, heart and vascular, stroke, inpatient
and outpatient surgery, orthopedics and labor and delivery,
including a NICU unit. Outpatient services include over a dozen
specialties and we offer senior living services. Adventist Health
and Rideout serves the community with 365 physicians, 2,745
associates and contractors, including 849 nurses.
Provides services to patients and their families to deal with needs
incidental to their hospitalization. Provides counseling, support
and assistance in making transition plans. Provides direction for
routine aspects of non-medical problems of patients and their
families. Assists with the planning and post-discharge of patients
with outside agencies, nursing staff, and relatives.
Education and Work Experience:
- Master's Degree in social work: Required in CA and HI
- One year experience in an acute or long term medical setting:
- Two years' experience in an acute or long term medical setting:
- Collaborates and facilitates patient discharge plans for those
requiring post Hospice or post inpatient care.
- Assists patient/family/caregiver in determining appropriate
level of Hospice care. Refers to outpatient resources as
- Initiates a plan of care and appropriate patient teaching.
Reassesses plan of care according to the physical and emotional
needs of patient.
- Develops Plan of Care (POC) based on patient assessment
including, psychosocial factors that would impede the POC, clinical
knowledge of appropriate theories of stages of human development,
the impact of terminal illness, mental status and family
- Performs other job-related duties as assigned.
