Sierra Vista Hospital is seeking a Therapist for our Outpatient

Sierra Vista Hospital location, a 171-bed psychiatric care facility
located in south Sacramento, is a full-service mental health
facility and a center of support for patients and their families.
Since 1986, we have offered our clinical expertise to those
suffering from emotional, behavioral and addiction problems. The
Therapist is responsible for providing the treatment activities to
the patient, which may include individual, family, group or couples
therapy.
Qualifications
Education: Master's degree from an accredited college or university
in social work, mental health or a related field
preferred.Experience: A minimum of one (1) year direct clinical
experience in a psychiatric or mental health setting preferred.
Experience in patient assessment, family motivation, treatment
planning and communication with external review organizations or
comparable license preferred.Licensure: LCSW, MFT, LPCC or License
Eligible (for Adult programs)Additional Requirement: CPR
certification Utilize clinical skills to ensure that all therapy
conducted is appropriate to the patient population.Complete all
psychosocial assessments on assigned patients in a timely
manner.Ensure that patient's rights are upheld.Advocate for the
lowest level of care consistent with patient needs.Function as a
resource for patient/families/significant others.High social-risk
case finding, required reporting and coordination/communication
with other involved agencies. DISCHARGE PLANNINGWork with the Case
Manager to develop clinically appropriate discharge plans.Provide
family meetings to discuss appropriate discharge planning.Organize
the use of resources to keep the patient as close to home as
possible, provide liaison to aftercare providers and follow up
after discharge to ensure efficacy of the discharge plans.
TREATMENT PLANNINGParticipate in the implementation of the
treatment plan and conduct an ongoing review of the plan to ensure
effective implementation of all treatment interventions.Provide
support and assistance to the patient and the patient's support
system.Work with the Case Manager to formulate an initial discharge
plan from information gathered during the assessment
process.Planning/assist in an episode of care to identify services
that will be provided if the patient is moved to a greater or less
intensive level of care depending on the needs of the case.Provide
feedback to the attending practitioner and treatment team members
concerning continuing certification of days/services. Sierra Vista
