Job Description:
The Clinical Quality Improvement Specialist (SQIC) reports to
the Quality Improvement Manager and will collaborate with medical
staff and the Quality Improvement Team to improve the health
outcomes of patients through panel management and care
coordination. They are also responsible for developing and
implementing quality improvement activities, analyzing data,
fulfilling documentation and reporting requirements for the Quality
Management program, developing educational and informational
materials, promoting health issues, and providing internal and
external customer support. The main responsibilities of this
position include assisting in completing required grant activities,
assisting patients in accessing all levels of care, improving the
quality of chronic care services, assisting medical care providers
in chronic disease management, and participating in continuous
quality improvement. The Clinical Quality Improvement Specialist
will also work on HEDIS/UDS/GPRA improvements in collaboration with
the QI manager and Medical Leadership.
