Job Description:

The Clinical Quality Improvement Specialist (SQIC) reports to the Quality Improvement Manager and will collaborate with medical staff and the Quality Improvement Team to improve the health outcomes of patients through panel management and care coordination. They are also responsible for developing and implementing quality improvement activities, analyzing data, fulfilling documentation and reporting requirements for the Quality Management program, developing educational and informational materials, promoting health issues, and providing internal and external customer support. The main responsibilities of this position include assisting in completing required grant activities, assisting patients in accessing all levels of care, improving the quality of chronic care services, assisting medical care providers in chronic disease management, and participating in continuous quality improvement. The Clinical Quality Improvement Specialist will also work on HEDIS/UDS/GPRA improvements in collaboration with the QI manager and Medical Leadership.
Essential Functions:

  • Perform routine clinical tasks
  • Utilize I2I and other tools to identify and manage Native high-risk, complex needs, and other persistently high utilization patient populations.
  • Assist primary care providers and quality improvement programs in formulating processes to identify and address unmet care needs. These processes might include PDSA cycles that include outreach, medical interventions, and follow-up evaluations.
  • Daily outreach and follow-up with patients via phone, letters, member portal messaging, and face-to-face interaction in order to coordinate patient care, schedule appointments, implement action plans and communicate PCP recommendations to patients.
  • Maintain an accurate patient registry via bi-monthly outreach and lost-to-care follow-up in order to provide current and accurate data.
  • Promotes a Team-Based Healthcare approach through the ongoing support of core principles.
  • Monitor tasks in NextGen and complete them within 48 hours
  • Maintain an awareness of the cultural and spiritual needs of the American Indian/Alaskan Native populations
  • Assists in providing education as appropriate
  • Maintain the highest respect for confidentiality in order to maintain patient privacy and sensitivity
  • Abide by industry best practices (e.g. proper PPE, ergonomic posture, etc.) to maintain a safe working environment; report safety issues or concerns to the Medical Clinic Manager or the Safety Officer immediately; review safety and health plan, understand and ask questions regarding safe practices in the work area.
  • Participates in department team huddles
  • Utilize HEDIS/UDS gap in care reports from health plans for patient outreach
  • Assist in panel management
  • Attend monthly/quarterly meetings with health plans in tandem with the QI Manager
  • Assists in the coordination of health care events
  • Compliance with all state and federal laws and regulations, as they pertain to position including; HIPAA, Sexual Harassment, Scope of Practice, OSHA, etc.
  • Other duties as assigned
    RequirementsMinimum Qualifications:
    • 1-year experience working in a health care setting
    • Experience in customer service
    • Experience with Microsoft Excel/PowerPoint and other Microsoft productsPreferred Qualifications:
      • Experience with NextGen/I2I or other electronic health record systems
      • Knowledge of traditional, cultural, and spiritual practices of the diverse AI/AN community, as well as the ability to work with other racially, culturally, ethnically, and financially diverse populations.
      • Bilingual
      • Medical Assistant certificate from an accredited institution
      • Experience conducting patient outreach to assist in the coordination of healthcare appointments and services
      • Experience with HEDIS/UDS/Population Health and panel management
