Job Description:
Position Overview:
Under the supervision of the Director of Patient Care Services
(DPCS), the Admissions Nurse, in cooperation with Hospice
Administrator and the Hospice Medical Director facilitates the
admission of patients to hospice services in the patient's place of
residence in alignment with SCAH, mission, goals and objectives.
The Admissions Nurse is responsible for conducting an initial
nursing assessment of a potential hospice patient and reporting the
patient's condition to the Hospice Medical Director using the
applicable Local Coverage Determinations. If the Hospice Medical
Director supports the hospice diagnosis and gives verbal
certification of terminal illness, the Admissions Nurse will admit
the patient to hospice services and attend to the immediate needs
of the patient. The Admissions Nurse will document the assessment,
interventions and outcomes in the patient's chart utilizing the
electronic medical record at the point of care.
Qualifications:
Education / Licensure / Certification
Graduate from an accredited school of nursing with a current
California Registered Nursing License - required
California Public Health Certification, OR - preferred
Completion of Community Health Nursing Program - preferred
Registered Nurse, State Licensure - RN - required
BLS - required
Driver License, valid and in State DL number
Automobile Insurance - required
Experience
One (1) year of acute medical experience - required
Direct patient care experience within the last (5) five years -
required
Experience with admissions or re-certifications under Region IX
local coverage determiantions (LCDs) - strongly preferred
Skills and Knowledge
Possess critical thinking skills, in-depth knowledge of clinical
disease entities common to hospice and palliative care for
terminally ill patient.
Must have the ability to function independently and as a member of
an interdisciplinary team; ability to exercise independent, sound
nursing judgment in planning and providing patient care
Ability to maintain harmonious constructive relationships with
internal and external customers.
Productivity standards - ability to meet productivity
expectations.
