Overview:Community Medical Centers has been providing central California with the highest level of care available in the region. Community Regional Medical Center measures up with top hospitals nationally and is the 3rd largest in California! With 685 beds, we are home to the only comprehensive Burn and Level I Trauma center between Los Angeles and Sacramento. The 56,000 square-foot emergency department is one of the largest and busiest in California! We have an 84-bed Level III NICU and plan to significantly expand pediatric specialty care. Our hospital serves 2.5 million people within 9 counties, averaging a daily census of 641 patients. We're affiliated with UCSF as a teaching facility; trained 3,000 Medical Doctors to date plus continuous invests in research and training grants.Responsibilities:The Clinical Nurse Specialist (CNS) is a leader and facilitator of change, coordinator of specialized care, and implementer of evidence-based care within/between organizations to facilitate quality improvement, patient safety, and lower healthcare costs. Clinical practice is consistently directed toward achieving quality, cost-effective, patient-focused outcomes across all three spheres of impact, which include: Patient Direct Care, Nurses and Nursing Practice, and Organizations and Systems. Patient Direct Care: The CNS will provide direct interaction with patients, families, and groups of patients to promote health and/or well-being and improve quality of life. This is characterized by a holistic perspective in the advanced nursing management of health, illness, and disease states. Nurses and Nursing Practice: CNS practice is the translation of advanced clinical expertise, expert knowledge, complex decision-making skills, and clinical competencies necessary for expanded practice to directly provide and influence care and outcomes of individuals, categories of patients, and/or communities. The CNS also advances nursing practice to achieve optimal outcomes by ensuring that nurses and associated interdisciplinary personnel utilize evidence-based practices to meet the multifaceted needs of patients and/or populations. Organizations and Systems: CNS practice transforms systems (such as healthcare institutions, political systems, and public and professional organizations) to mobilize and change through expertly designed and implemented nursing interventions. The CNS is a leader of change in healthcare organizations, developers of scientific evidence-based programs to prevent avoidable complications, and coaches of those with chronic diseases to prevent hospital readmissions. Clinical Nurse Specialists are facilitators of interprofessional teams in acute and chronic care facilities to improve the quality/safety of care and patient outcomes, including prevention of hospital-acquired conditions, reducing lengths of stay, and preventing hospital readmissions.Accountabilities1. PATIENT DIRECT CARE2. Uses relationship-building communication to patients, families and caregivers to promote health and wellness, healing, self-care, and peaceful end-of-life, as applicable.3. Conducts comprehensive health assessments in diverse care settings including psychosocial, functional, physical, and environmental factors.4. Creates evidence-based, cost-effective interventions, to meet the multifaceted needs of complex patients.5. Designs and employs educational strategies to patients, families and caregivers that consider readiness to learn, complex learning needs, individual preferences, atypical responses, and other social determinants of health.6. Provides expert patient/caregiver-centered consultation based on a broad range of theories and evidence for patients with complex healthcare needs. Is also able to use advanced communication skills to navigate difficult conversations. As appropriate, facilitates patient and family understanding of the risks, benefits, and outcomes of proposed healthcare regimens to promote informed, shared decision-making.7. Evaluates impact of nursing interventions on patients' aggregate outcomes using a scientific approach.8. Leads and facilitates coordinated care and transitions in collaboration with the patient and interprofessional team.9. Facilitates resolution of ethical conflicts in complex patient care situations, including impact of scientific advances, cost and clinical effectiveness, and influence on patient and family values, preferences, and rights.10. NURSES AND NURSING PRACTICE11. Provides expert specialty consultation to nurses related to complex patient care needs.12. Promotes nursing interventions for clinical staff that alleviate ethical conflicts, moral distress, and the impact of implicit bias on relationship building and outcomes.13. Assesses the nursing practice environment and processes for improvement opportunities, using evidence-based knowledge as a foundation to achieve optimal nurse-sensitive outcomes.14. Fosters a healthy practice environment by exhibiting positive regard, conveying mutual respect, acknowledging the contributions of others, and employing positive negotiation skills to mediate conflict management.15. Mentors nurses and clinical staff in using evidence-based practice principles, in order to foster knowledge, skills and professional development. Uses reflective practice activities that promote self-awareness and invites peer feedback to improve the practice of nursing.16. Leads nurses in the process of planning, implementing, and evaluating change, considering intended and unintended consequences.17. Mentors nurses to analyze and understand legislative, regulatory, and organizational policies and procedures that affect nursing practice and patient outcomes.18. Evaluates the outcomes of nursing practice using methods that provide valid data.19. ORGANIZATIONS AND SYSTEMS20. Uses leadership, team building, negotiation, collaboration, and conflict resolution to build partnerships within and across the organizational system and/or communities.21. Consults with interdisciplinary healthcare team members to integrate patient needs, preferences and strengths into healthcare plan(s) to optimize clinical outcomes and the patient experience.22. Leads and participates in systematic quality improvement and safety initiatives, based on precise problem/etiology identification, gap analysis, and process evaluation.23. Provides leadership within the interprofessional team in identifying, translating, developing, implementing, evaluating and disseminating evidence-based practices and research opportunities.24. Leads and participates in the process of selecting, integrating, managing, and evaluating technology and products to promote safety, quality, efficiency, and optimal health outcomes.25. Demonstrates stewardship of human and fiscal resources in decision-making.26. Disseminates CNS practice and clinical outcomes to internal stakeholders and the public.27. Promotes nursing's unique contributions to advancing health to stakeholders through policy and organizational projects.28. Advocates for equitable healthcare by participating in professional organizations, community outreach and/or public policy activities.Qualifications:Education Master's Degree in Nursing required Graduate of a CNS program accredited by the National League of Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE) required Doctoral Degree in Nursing preferredExperience 2 years of clinical or education experience in area of specialty requiredLicenses and Certifications RN - Current State of California Registered Nurse license required CNS - Clinical Nurse Specialist required BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required NATIONAL - National Certification related to specialty and applicable to role preferred Additional certifications may be required by certain specialties/departments Please note: Per the CDPH order, all healthcare workers are required to be vaccinated for COVID-19 or have an approved exemption and test regularly.
