Job Description:

The Hospice Administrator serves as the leader and expert in hospice services. The position is responsible for operational planning and directing the development, implementation, and improvement in services within a given geography. The position ensures that consistent processes and program objectives extend across the continuum of care, in alignment with SCAH goals and objectives and those of the regional affiliates. The position effectively collaborates with hospital and foundation (medical group) affiliates to ensure efficient and effective processes, alignment of goals and objectives and shared outcomes. The position oversees Hospice operations and is responsible for all aspects of its operations, quality management and financial performance. The Hospice Administrator is responsible for planning, organizing, and directing the Hospice program for the assigned area in accordance with program requirements and regulatory or accreditation standards. This position is responsible for collaborating and supporting other engaged leaders, such as the Home Health Administrators, AIM staff and Supervisors, to achieve program goals and objectives. The Hospice Administrator leads all aspects of the program, provides guidance and /or direct intervention in resolving challenging situations. This position will coordinate with leadership within SCAH and at affiliates to ensure that patients are receiving the specified program services in a high quality and cost-effective manner. This position is responsible for developing and promoting Hospice services throughout the assigned area. The Hospice Administrator works closely with affiliates (hospitals, physician groups, and medical foundations) to provide education, identify and resolve process issues, and to develop and improve outcomes of care. The Hospice Administrator supports leadership with strategic planning and initiatives related to AIM, palliative care, and end of life care at a SCAH level and a system level. It supports regional and affiliate organizational business processes through collaborative partnerships with regional and affiliate level operational leaders. Assigned areas for this position will include 1 hospice branch operation, unless otherwise designated by SCAH Leadership. Education BA/BS Business, Health Care, Nursing, or related field required. Masters in clinical area or management desired. Licensure Valid California driver's license; current auto insurance; current BCLS/CPR certification Required. Current California RN license desired. Experience/Knowledge/Skills * Minimum of 5 years relevant and progressive management experience to include at least 3 years experience in management hospice. Palliative Care experience also preferred. Other home care based experience may be considered. * Minimum of 5 years of direct patient care of patients with advanced chronic illness, palliative care, or hospice care and familiar with care management principles, psycho-social, spiritual and cultural aspects of care * Ability to demonstrate communication, presentation, leadership, analytical and problem solving skills * Demonstrated ability to lead and manage change through the use of interpersonal skills such as negotiating, collaborating a influencing * Demonstrated ability to achieve results on both a strategic and operational level * Demonstrated expertise in program management * Ability to be a strong and consistent mentor to staff and to use disciplinary process appropriately * Strong interpersonal skills to navigate sensitive issues and conflicts with or among others, and respectively direct and reinforce staff efforts * Excellent understanding of hospice and palliative care services, Title XXII, Medicare Conditions of Participation, home care, quality management and PI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payers; good working understanding of fiscal responsibilities, budget and financial reports. Knowledge of available healthcare and community resources appropriate for population served. Knowledge of home based clinical practice and risk management as applicable to population and setting.
