Job DescriptionThe primary focus of the Emergency Medicine Physician is to provide high-quality care to serve the extensive needs of chronically ill, frail, and vulnerable patients of the community. The Physician must be eager to work with a diverse, multidisciplinary team while remaining dedicated to transforming healthcare.Details:

• Permanent, full-time position
• Function as day-to-day clinical leader
• Provide decision support to nurse practitioners and directing the multidisciplinary team
• Perform four to six preventive visits daily to optimize chronic conditions, assess home environment, educate patients and caregivers, and develop proactive care plans
• Perform urgent care visits in the home to avoid unnecessary ED transfers and hospital admissions
• Assume responsibility as primary care provider in situations where there is no existing PCP
• Coordinate with other physicians across the continuum of care
• Coordinate and offer medical direction to community-based organizations

    • Doctor of Medicine or Doctor of Osteopathy from an accredited educational institution
